Take Care Clinic is available monthly for free care, and we will reassess at mid year to see how we want to move forward with providing care.
You will find the Take Care Health and Wellness Application attached. Completion of this application will enable us to care for you through Take Care Health and Wellness.
We understand your desire for privacy. Accordingly, except for verification purposes, the information included in your application will be treated as confidential information. It will only be shared within Take Care Health and Wellness on a need-to-know basis.

All parts of the form must be filled out to be considered for our Take Care Clinic option.
We have a limited number of funds each month and while we aim to serve all who need care, we have capacity for up to 20 people currently.
We ask for a donation to the non profit, suggested amount of $20. We know this may not be feasible for all, and understand if you are not able. We ask that you share our mission, in hopes that we can spread word of our work.
